We are open on:

• Monday: 7:00am-5pm

• Tuesday: 7:00am-5pm

• Wednesday: 7:00am-5pm

• Thursday: 7:00am-5pm

• Friday: 7:00am-2pm

Our clinic is closed everyday from 12 to 1pm for lunch except Fridays. Our endoscopy center is open from 7 am Mon to Thurs until we finish cases.
 

To make appointments please call 910-353-6158 at any of the above times. If we are out to lunch please leave a voice-mail and we will return your call. Please print out, complete and bring in this New Patient Form to help expedite your visit to our office.

If we had seen you in the past but not within the last 12 months, please print, complete, and bring along the Established Patient Form.
